SYDNEY DIABETES HEALTH ASSESSMENT UNIT


The Sydney Diabetes Health Assessment Service was opened in July 2002 to help general practitioners manage patients with diabetes.

What does it do?
The service is a specialist unit for people with diabetes who live in the catchment of the Northern Sydney Area Health Service. It provides an annual assessment or check-up for patients with either Type 1 or Type 2 diabetes, focusing on key risk factors for the development of complications.

Patients can be newly diagnosed or have had diabetes for many years.

Why is an annual assessment important?
Patients with Type 1 or Type 2 diabetes should have a full assessment once a year to ensure there is no damage to important body systems caused by their diabetes.

What does the assessment involve?

Eye examination
Visual acuity test
Dilatation of the eye
Retinal eye photography
Examination of the retinal eye photography by an ophthalmologist

Foot examination
Visual assessment of the foot
Neurological testing
Assessment of blood circulation in foot using a Doppler

Kidney function assessment
Blood sample
Urine sample

Assessment for autonomic neuropathy
Examination using a pupilometer

Heart and blood pressure examination
Resting cardiograph
Blood pressure
Blood sample

We also review the patient’s medications, including diabetic medications, blood pressure medications and cholesterol medications.

How does it work?
A diabetes clinical nurse consultant will interview the patient. This is followed by a consultation with an endocrinologist to provide an assessment and advice on management.

Referrals are organised if required to an ophthalmologist; experienced podiatrist and/or dietician.
A referral to the High Risk Foot Service or the Diabetes Education Centre can also be organised if necessary.
Interpreters can be provided if this is arranged when the appointment is made.
